We’re so glad you’re part of our Crystal Stairs network. To help you with submitting your timesheets, we have uploaded the following Attendance Record template in English and Spanish.

As of June 1, 2025 we will no longer be printing attendance records. You may download them here:

If you do not have printing capabilities at home, you may request that we mail you copies.

To save you time, please note the following options for submitting your attendance sheets.

*Please note that parking and idling is not permitted in front of the building.

Attendance Sheet Submission Options:

1. Mail your completed attendance sheets to us: Crystal Stairs Provider Services, P.O. Box 92240, Los Angeles, CA 90009-2240
2. Drop off attendance sheets at either of our two Crystal Stairs drop box locations.
   1. In Los Angeles, you may drop off at the turnabout at the S Fairfax Ave. entrance to the Wateridge campus.
   2. In Long Beach, you may drop off attendance sheets right outside the building at 4300 Long Beach Blvd, Long Beach, CA 90807
3. Support during “busy week.” During the first week of each month, Crystal Stairs staff will be stationed at peak times in the turnabout area to help streamline drop-offs.

Blank Attendance Sheets 

• As of May 2025, replacement or blank attendance sheets will no longer be distributed at the front desk. Instead, they will be available for download on our website for your convenience as needed.
